Life Connections™ Program at The Pavilion
The Life ConnectionsTM Program provides specialized care and services for residents who require a higher level of assistance than is typically offered in an assisted living setting. Residents generally have dementia and related disorders such as Alzheimer's Disease. Residents enjoy all of the care, services and amenities provided in our Levels of Care Program, plus increased staff attention in a safe, monitored and supportive environment. Specialized care and daily structured programming are provided to meet the unique needs of Pavilion residents. A customized activities program emphasizes each resident's strengths, promotes independence, and provides opportunities for socialization. The Life ConnectionsTM Program has been developed to enhance the quality of life and maintain the dignity of each resident.

As in our assisted living program, we also offer a Levels of Care Program for our Life Connections residents. This program helps insure that residents receive the care and attention they require. Your community representative will gladly review the Life Connections Level of Care Program with you.

*The Level of Care assessment scale and monthly charges may be adjusted from time to time to better reflect the care needs of our residents and to help insure that the community has all the resources available to meet those needs
